The parties agree as follows:

1. DESCRIPTION OF SERVICES. Beginning on the above stated date, Credit Correction Service will provide the following services (collectively, the "Services"):

A. "Credit Rebuilding" Program. One year of Credit Counseling, available by telephone or e-mail, Monday- Friday, 9:00 a.m. - 5:00 p.m. (Eastern Time). One year of Credit Monitoring, a Textbook/Manual, and access to our On-line Resource Centers.

B. "Credit Restoration" (Repair) Service. YOU MUST BE ENROLLED AS A CREDIT REPAIR FOUNDATION MEMBER TO USE THE NO COST CREDIT REPAIR SERVICE. Assist the client(s) in obtaining copies of all three credit reports from the reporting agencies. Review, identify and discuss inaccurate and erroneous entries on said reports to determine the best cause of action for removal or improvement of account information, with the purpose of improving the client’s credit rating. Drafting letters to the credit reporting agencies disputing the negative or erroneous information on client’s reports. These letters have been reviewed by an attorney and contain all the statues from the Fair Credit Reporting Act to insure that the credit agencies will not disregard the dispute as frivolous. Drafting letters to client’s creditors citing the Fair Debt Collections Practice Act, drafting Lawful Notices, assist the client(s) in arranging settlements with creditors if applicable or necessary. (Other fees may apply.) Communicate with your creditors and the credit reporting agencies. Where necessary, add an up to 100-word statement to accompany your credit reports.

2. CUSTOMER TERMS AND CONDITIONS. The client(s) agrees to: (a) Purchase the most recent copies of their credit reports; (b) Provide Consultant with a copy of all correspondences received from the credit reporting agencies or creditors relating to credit report entries; (c) Notify Consultant if you do not receive any correspondences in 45 days; (d) Not to contact the credit bureaus (Experian, Equifax, and Trans Union) for any reason without consulting Consultant; (e) Not to apply for any type of credit during the period of this agreement without consulting Consultant; (f) Forward all credit reports to Consultant as you receive them via US Mail. No updated credit reports will be accepted via fax. (There are too many pages and too much room for error.)

At the conclusion of each and every investigation prompted by our company, the credit bureaus will send the client(s) an updated/corrected copy of their credit reports (every 60 to 90 days). It is the client's responsibility to send these reports at their earliest convenience to our office in order that client's counselor may review these reports for progress made on client's behalf and to continue working on client's behalf.

3. GUARANTEE. Money Back Guarantee. If at the end of twelve months, you have forwarded credit reports every ninety days and completed your commitments to the service, and you are not satisfied with our results, we will gladly offer you a refund. Consultant agrees to improve client(s) credit profile during the subscription period of one year. Client(s) understands that the results obtained by Consultant on behalf of client(s) are dependent on numerous factors, including but not limited to client(s) ability to repay debts and loans, cooperation of client(s) creditors, and credit bureaus ability to verify information provided to them by Consultant on behalf of client(s).

4. CREDIT REPAIR FOUNDATION MEMBERSHIP FEE. Total fee is $1,295.00 for an individual person, $1,595.00 for a couple. Payments may be made by check or credit card.

5. CONFIDENTIALITY. The client(s) recognizes that Consultant has and will have information which is valuable, special and unique assets of the client(s) and needs to be protected from improper disclosure. In consideration for the disclosure of the Information, the client(s) agrees that Consultant will not at any time or in any manner, either directly or indirectly, use any Information for Consultant's own benefit, or divulge, disclose, or communicate in any manner any Information to any third party without the prior written consent of the client(s). Consultant will protect the Information and treat it as strictly confidential. A violation of this paragraph shall be a material violation of this Agreement.

6. CONFIDENTIALITY AFTER TERMINATION. The confidentiality provisions of this Agreement shall remain in full force and effect after the termination of this Agreement.

7. ENTIRE AGREEMENT. This Agreement contains the entire agreement of the parties and there are no other promises or conditions in any other agreement whether oral or written. This Agreement supersedes any prior written or oral agreements between the parties.

8. SPECIAL POWER OF ATTORNEY: (needed in order to successfully complete the Credit Restoration process)
Be it known that I/WE, the undersigned, hereby appoint Credit Correction Services; c/o Credit Repair Foundation; 2701 N. Rocky Point Drive, Suite 995; Tampa, Florida 33607; Toll Free: (866) 899-4663  Telephone: (813) 637-9797  Fax: (813) 637-9798; E-Mail: CustomerService@CreditRepairFoundation.com, as my Attorney-in-Fact ("Agent").

My agent shall have full power and authority to act on my behalf but only to the extent permitted by this Special Power of Attorney. My Agent's powers shall include the power to:

Sign my name on all documents written on my behalf for the purpose of challenging and verifying account information on all of my consumer credit reports.

9. INDEMNIFICATION. The client(s) agrees to indemnify and hold Consultant, their agents, brokers and employees, harmless from all claims, losses, expenses, fees including attorney fees, costs, judgments that may be asserted against the client(s) that result from the acts of omissions.

10. CONSUMER CREDIT FILE RIGHTS UNDER FEDERAL AND STATE LAW. You have a right to dispute inaccurate information in your credit report by contacting the credit bureau directly. However, neither you nor a "credit repair" company or credit repair organization has the right to have accurate, current and verifiable information removed from your credit report. The credit bureau must remove accurate, negative information from your report only if it is over 7 years old. Bankruptcy information can be reported to 10 years.

You have a right to obtain a copy of your credit report from a credit bureau. You may be charged a reasonable fee. There is no fee, however, if you have been turned down for credit, employment, insurance, or a rental dwelling because of information in your credit report within the preceding 60 days. The credit bureau must provide someone to help you interpret the information in your credit file. You are entitled to receive a free copy of your credit report if you are unemployed and intend to apply for employment in the next 60 days, if you are a recipient of public welfare assistance, or if you have reason to believe that there is inaccurate information in your credit report due to fraud.

You have a right to sue a credit repair organization that violated the Credit Repair Organization Act. This law prohibits deceptive practices by credit repair organizations.

You have the right to cancel your contract with any credit repair organization for any reason within 3 business days from the date you signed it. Credit bureaus are required to follow reasonable procedures to ensure that the information they report is accurate. However, mistakes may occur. You may, on your own, notify a credit bureau in writing that you dispute that accuracy of information in your credit file. The credit bureau must then reinvestigate and modify or remove inaccurate or incomplete information. The credit bureau may not charge any fee for this service. Any pertinent information and copies of all documents you have concerning an error should be given to the credit bureau. If the credit bureau's reinvestigation does not resolve the dispute to your satisfaction, you may send a brief statement to the credit bureau to be kept in your file, explaining why you think the record is inaccurate. The credit bureau must include a summary of your statement about disputed information with any report it issues about you.

NOTE: The State Law of Florida provides 5 days after signing your contract for you to cancel your contract with the Consultant. Neither the Consultant nor the Credit Repair Foundation have any restrictions on cancellation and you may cancel your service with the Consultant or the Credit Repair Foundation at any time.

Signing and submitting this form constitutes acceptance of all terms and conditions of this contract.

“ Notice of Cancellation”
You may cancel this contract, without any penalty or obligation, within three business days after the date the contract is signed. To cancel this contract, mail or deliver a signed, dated copy of this cancellation notice, or other written notice to: Credit Correction Services  c/o Credit Repair Foundation  2701 N. Rocky Point Drive, Suite 995  Tampa, FL 33607 not later than midnight three (3) business days from the date of this contract.
